Essential Living Documents Package (Individual)

Legal DIY is a package of “a la carte” legal help that we have developed to serve our communities and bring greater access to certain estate planning services. This package includes:

  • A toolkit of downloadable living documents (Advanced Directive, Medical Power of Attorney, and Durable Power of Attorney) for your use
  • An Instructional Guide, and
  • A 15-minute review meeting with member of the firm
  • What are Living Documents?

    Advance Directive/Living Will

An Advance Directive covers the administration, withholding, or withdrawal of life-sustaining procedures when you have a terminal condition and are unconscious or otherwise incompetent. In this very limited set of circumstances, declarations you make in a properly executed Living Will as to artificial nutrition, artificial hydration, and the administration, removal, or refusal of life-sustaining procedures govern your treating physician's course of action.

    Medical Power of Attorney

A Medical Power of Attorney allows you to name an agent who will make health care decisions for you when you are not able to do so. In the document, you can give specific instructions to your agent about various issues like surgery, medical treatment, or the need for nursing home care.

    Financial Power of Attorney

A General Durable Power of Attorney is a method of giving another person (agent) the legal authority to manage some or all your financial affairs. The agent can do whatever the principal may do — withdraw funds from bank accounts, trade stock, pay bills, cash checks — except as limited in the power of attorney.
